I, by this form, hereby exempt and release Div'Ocean Bonaire staff and all related entities from all liability or responsibility whatsoever for any injury, property damage, or wrongful death, however caused, including but not limited to the negligence of the released parties, whether passive or active, that may occur to me of my family, heirs or assigns, as a result of my participation in this experience. I also affirm that I do not have any physical or mental impedance that can risk myself or third parties during the dive experience and that I am a certified scuba diver that thoroughly understands the potential risk and inherent hazards of scuba diving. I also hereby verify that all of the equipment I am renting is in good working condition and that I am responsible for it.
